Tube Fasteners are most often used in a critical care / clinical setting. As such, there is no separate reimbursement for the products, the cost is bundled into the Prospective payment system and based on the diagnosis and care needs of the patient.

A9999 Miscellaneous DME Supply or Accessory, Not Otherwise Specified
B9998 Not Otherwise Classified for Enternal Supplies
If these products are prescribed for home use and obtained through a DME supplier, the above HCPCS codes will be used for billing. If the HCPCS code is a miscellaneous code, information about the product will need to accompany the bill. The information should include the manufacturer name, the manufacturer's product code and name, a detailed description of the product, and the use and medical necessity for the product.
HCPCS code A5200 is contained in both the Urologics Policy and the Enteral Nutrition Policy. Per the Medicare policy for Urologic Supplies, a catheter/tube anchoring device (A5200) is covered and separately payable when it is used to anchor a covered suprapubic tube or nephrostomy tube. If code A5200 is used to anchor an indwelling urethral catheter, payment will be based on the allowance for the least costly medically appropriate alternative, A4333.
Per the Medicare policy for Enteral Nutrition, payment for a catheter/tube anchoring device is considered included in the allowance for enteral feeding supply kits (B4034-B4036). Code A5200 should not be billed separately and is not paid in addition to the supplies for enteral nutrition.
